Your Visit

Enter our office to a pleasant and comfortable waiting room. If you have filled out the new patient or patient update forms obtained from the web, please let the receptionist know. If not, you will be given these forms to complete upon your arrival. A separate copy of these forms will be required for each patient. Forms you will need to complete for a first visit include the patient questionaire, parental consent form and our financial policy. For children under 18, please complete a parental consent form as well.

Our staff will require a copy of your driver's License.  For insurance patients, we will also require a copy of your insurance card.
For those who will be regular patients, we may request a photo for your file, as well.  While waiting for a free operatory, you can read magazines or watch TV in our waiting room.  Complimentary water and tea are available for your wait.
Once an operatory is ready, you wlil be called back for your appointment.  A few family members are welcome to come with you during the exam or procedure.  However, if you are gettting X-Rays, family members must remain in the waiting room until the staff finishes taking the X-Rays.  You will be informed when you can join your family member in the operatory.
